Kent LD Awards are back - only 3 weeks left to make your nominations!

The closing date for the awards is 5pm, Friday 22 July 2016.

How to enter

Choose the category you want to make a nomination for. If you are entering for more than one category, please fill in a separate form.  You cannot nominate yourself.

Nominations are invited under the following categories:

1. Employer of people with a disability award

Do you know an organisation who employs someone with a learning disability. Making sure they are treated the same as everyone else

2. Supported housing award

Do you know someone or an organisation who give good information and advice on housing, or give a good place to live for people with a learning disability.

3. Supporting people with learning disabilities award (team or individual)

Do you know a nurse, social worker, occupational therapist or support worker (etc) who gives excellent service to people with a learning disability

4. Citizenship award (opening up communities) This year’s theme for Learning Disability Week is friendships and relationships. With this in mind do you know a service, organisation or person who has worked to make a real difference in their community. This work will have helped people with a learning disability feel included.

5. People’s award

Do you know someone who through their hard work has made a good difference to their own and or other people’s lives.

6. Helping people stay healthy award

Do you know someone who has helped a person with a learning disability get healthier? This award is to celebrate outstanding staff/carer(s)/organisations or projects which have shown exceptional assistance to helping people stay healthy.
